If eligible, you'll be able to download and print the PPSPM form
You then need to complete Section 1 and send the form to your previous public sector pension administrator.
Your previous public sector pension administrator receives the PPSPM form where they complete Section 2
They return the completed form to MyCSP so that we can progress your application.
